Carers Register

Carers_Wiltshire

www.carersinwiltshire.co.uk

The Tolsey Surgery carers register is used to identify people who give care or support to someone who is ill, frail, disabled or is mentally ill so that we can offer them the support they need and refer them to Carer Support Wiltshire.

Who is a carer?

A carer is someone who provides help and support to a partner, relative, child, friend or neighbour who could not manage without their help.

Carers help the people they care for because they may be ill, frail, disabled, have mental health or substance abuse problems.

Tolsey Carers Register

If you are a carer please join the register, we can offer you:

  • Priority flu vaccination
  • Flexibility when making appointments
  • Joint home visits with the person you care for

We will provide you with a carers pack.  This contains a referral to Carer Support Wiltshire which is an independent charity established to provide support to carers living in Wiltshire (see our noticeboard for further information)

If you are a carer please contact Lisa Neal (Carers Lead) at the surgery and she will provide you with a carers pack and help you with your referral or if you prefer she can complete the referral on your behalf.
